Information & Forms
Your First Visit
You can expect a full evaluation of your injury or condition during your first visit to Vineland Physiotherapy.
Under normal circumstances your first therapy will also be delivered at that time so please wear loose-fitting, comfortable clothing that is appropriate for the injured or painful area of the body to be assessed and treated.
Bring with you a list of any prescribed or over-the-counter medicine that you take regularly, as well as any notes from your doctor and any diagnostic test results that you have available.
To speed up the paperwork, please download and print forms from this site and bring them to your appointment.
What to Wear
When coming to your physiotherapy appointment, make sure that you dress with comfort and mobility in mind. Loose-fitting clothes such as tank tops, t-shirts, and shorts or jogging pants allow your physiotherapist to access your injured area to assess and treat your injury properly.
If you have an upper-body injury, such as a neck or shoulder injury, sleeveless shirts are best. If you have a knee or ankle injury, shorts or loose jogging pants are ideal. Comfortable, supportive, and non-slip shoes are essential for physiotherapy as they help to keep you stable and safe.
Lastly, long hair should be tied up, and jewelry should be removed before your treatment session.
The goal of dressing appropriately for physiotherapy is so that your physiotherapist can provide you with the best treatment while keeping you comfortable.
